Punctured lung
(Tension pneumothorax): A 25-year-old man was brought to the
emergency department after having been hit in the left lateral
chest with a jet ski. He was short of breath and had a dangerously
low blood pressure. On examination, there was evidence of his
trachea being shifted to the right and absent breath sounds on
the left when the physician listened with a stethoscope. The chest
x-ray reveals a shift of the central structures of the chest
to the right and absence of lung markings in the left lung field
consistent with a punctured lung and air in the chest cavity that
is creating pressure on the great vessels leading to the heart.
This causes a dangerous decrease in blood return to the heart.
(tension pneumothorax). (L.S.)
