Acromio-clavicular (A-C) separations are usually due to a fall or direct impact onto the shoulder. Depending on the degree of ligamentous damage, they are graded from I to V in severity. Grade I is without deformity. Grade II involves rupture of the AC ligaments. Grade III involves further rupture of the coracoclavicular ligaments. Grades IV and V are characterized by posterior displacement and are quite rare.

Treatment is generally nonsurgical with the application of a simple sling. After several days, active range of motion exercises may be started as tolerated.
